What You Need for Your First Visit to Legacy Implant and Oral Surgery
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, surgery can be performed the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.
Please assist us by providing the following information at the time of your consultation:
- Your surgical referral slip and any x-rays if applicable.
- A list of medications you are presently taking.
- If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.
A preoperative consultation and physical examination is mandatory for patients undergoing IV anesthesia for surgery.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, osteoporosis, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, bisphosphonates, etc.).
X-Rays
If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.

Q: Do you charge for consultations?
A: Dental Implant consultations are complimentary.
Q: Do you take payment plans?
A: No, we do not. But we do accept Care Credit.
Q: How much is a procedure?
A: You need to be seen for a consultation to determine the cost.
Q: Can I take an Uber for my surgery?
A: No, you have to have a responsible person accompany you and over the age of 18.
Q: Do bills get filed to my dental or medical insurance?
A: We have to verify your insurance first, but typically it gets filed to your dental insurance.
Q: Can I pay a bill online?
A: Currently, we can only take payment by phone, a mail-in check, or money order.
Q: Are you open on the weekends?
A: We are open Monday-Thursday: 8 AM – 5 PM and Friday 8 AM – 3 PM.
Q: Can I schedule my consultation and surgery at the same time?
A: We need to see your for a consultation first then you can schedule your procedure.
Q: Do you take Medicare or Medicaid?
A: We are out of network with Medicare/Medicaid but we can still see you.
Q: Will you transfer my prescriptions to a different pharmacy?
A: We can change the pharmacy before your procedure but once they are sent we aren’t able to transfer them.
