Restoring Bone for Dental Implant Success

Successful implant placement depends on adequate bone density and volume in the jaw. For patients with insufficient bone in the upper jaw, sinus grafting (also called a sinus lift or sinus augmentation) is an option that can provide sufficient bone mass to support implants, ensuring stability and longevity.

What is Sinus Grafting, and Why is it Necessary?

Sinus grafting is a surgical procedure adding bone material to the upper jaw, specifically beneath the maxillary sinuses. The sinus membrane is gently lifted to create space for the bone grafting material, which integrates with the existing bone over time. This procedure is essential when the natural bone in the upper jaw is too thin or has receded due to tooth loss, gum disease, or the natural expansion of the sinus cavity.

Dental implants cannot be securely anchored without sufficient bone, increasing the risk of implant failure. Sinus grafting solves this problem by adding additional bone tissue and encouraging new bone growth. This provides a stable base for implants, allowing patients to enjoy functional, long-lasting dental restorations.

Why Might I Need Sinus Grafting?

  • Insufficient Bone Height: Loss of bone density in the upper jaw due to missing teeth or age.
  • Bone Resorption: Severe bone loss following tooth extraction or trauma.
  • Sinus Expansion: Natural sinus growth that reduces the available bone for implants.
  • Missing Teeth: You are missing teeth due to a congenital disability or genetic condition.

Ideal Candidates

Sinus grafting is typically recommended for patients seeking dental implants who lack sufficient bone density or height in the upper jaw. However, individuals with chronic sinus issues or active sinus infections may not be ideal candidates until these conditions are addressed. Dr. Stamboulieh evaluates each patient individually to determine whether they are an appropriate candidate for a sinus grafting procedure.

The Sinus Grafting Procedure

In the most common sinus augmentation procedure, a small incision is made on the premolar or molar region to expose the jawbone. A small opening is cut into the bone, and the membrane lining the sinus is pushed upward. The underlying space is filled with bone grafting material from your body or a cadaver. Sometimes, synthetic materials that can imitate bone formation are used. The incision is stitched up after the bone is implanted, and the healing begins. After several months of healing, the bone becomes part of the patient’s jaw, and dental implants can be inserted and stabilized in this new sinus bone.

If enough bone between the upper jaw ridge and the bottom of the sinus is available to stabilize the implant well, sinus augmentations and implant placement can sometimes be performed as a single procedure. If enough bone is not available, the sinus augmentation will have to be performed first, then the graft will have to mature for several months, depending upon the type of graft material used. Once the graft has matured, the implants can be placed.

Bone Grafting Materials

Various materials can be used for sinus grafting, each with its own benefits and limitations:

  • Autografts: Bone harvested from your own body, offering excellent compatibility but requiring additional surgery. Autografts are usually taken from the roof of the mouth.
  • Allografts: Bone from a human donor, widely used for safety and convenience and available from a bone or graft bank.
  • Xenografts: Bones from animal sources, such as bovine, are known to promote new bone growth.
  • Synthetic Grafts: Man-made materials designed to mimic natural bone.

Dr. Stamboulieh will discuss these options with you and determine which is the most appropriate for your needs.

Recovery After Sinus Grafting

After your sinus grafting procedure, you will experience some mild swelling, bruising, and discomfort, which can be managed with prescribed medications. Most patients resume normal activities within a week.

Post-Operative Care

  • Avoid blowing your nose or sneezing forcefully.
  • Refrain from smoking to promote healing.
  • Follow dietary guidelines provided by our office and attend follow-up visits.

Dental implants are usually placed 6–9 months after the graft, allowing time for the new bone to fully integrate.

Risks and Complications

While complications are rare, they can include infection, bleeding, or sinus membrane perforation. In very rare cases, graft failure may occur. However, Dr. Stamboulieh uses advanced techniques to minimize risks and addresses complications immediately, ensuring your safety and peace of mind.

Sinus Grafting Frequently Asked Questions

How long does the sinus grafting procedure take?

The surgery usually takes 1-2 hours, depending on the complexity.

Is sinus grafting painful?

The surgery itself is not painful. We numb the area, and the surgery is performed under local, oral, or IV anesthesia, depending on your preference.

Will I be in pain after the surgery?

After the surgery, you will feel some discomfort, but this is usually easy to manage with over-the-counter pain relievers. The discomfort and swelling should subside within a few days.

Do sinus grafts ever fail?

Very rarely. If sinus grafting does fail, Dr. Stamboulieh may repeat the procedure or explore alternatives with you.
